Most travelers from Canada with proof of full COVID-19 vaccination can enter Dominican Republic, but there are restrictions.

Travelers are not required to present a vaccination certificate to enter the Dominican Republic.

Mandatory travel declaration form before arrival
Travelers are required to fill out an electronic entry form before arrival, which combines the Traveler’s Health Affidavit, Customs Declaration, and International Embarkation/Disembarkation forms. Once completed, it will provide a QR code that will be scanned by the authorities on arrival. The form must be completed individually by each traveler and travelers must complete the form a maximum of 72 hours before arrival in the Dominican Republic. When traveling as a family, it is not necessary for each traveler in the same family to fill out an individual form. Travelers are required to fill out an electronic entry form before departure, which combines the Traveler’s Health Affidavit, Customs Declaration, and International Embarkation/Disembarkation forms.
